Contractor incident, near miss and hazard reporting

At Power and Water we capture health, safety, environmental and other incidents, near misses and hazards to identify and address gaps in our controls and processes.

Contractors must notify their Power and Water Representative verbally of incidents, near misses and hazards as soon as possible in accordance with our health, safety and environment requirements. This includes environmental incidents, no matter how big or small the impact is. Provide as much detail about the event as possible and submit additional information such as photos, when requested.

If your Power and Water Representative is not available, you should notify our Health, Safety and Environmental Services team on 8924 5199 or 0418 731 200 for urgent environmental events.

Contractors are required to report monthly in relation to works carried out for Power and Water by submitting the form below, including any notices or formal directives received from a regulatory body.

Definitions

  • Incident: Something that has happened and resulted in harm or damage.
  • Near miss: Something that has happened and didn’t result in harm or damage, but had the potential to do so.
  • Number of workers: Number of workers engaged to undertake works for Power and Water in the reporting period. Includes all casual and part time positions.
  • Hours worked: Total number of hours worked for the period by all employees undertaking works for Power and Water.
  • Lost Time Injury (LTI): A work related injury or illness certified by a medical practitioner that results in the absence from work for at least one scheduled day or shift.
  • Medical Treatment Injury (MTI): MTI is defined as a work related injury or illness that resulted in a certain level of medical treatment (not first aid treatment) provided by a physician or other medical personnel.
  • First Aid Injuries (FAI): FAI is defined by treatment administered by and within the qualifications of a trained first aid attendant or Occupational Health Nurse.
  • No Treatment Injury (NTI): Injury not requiring first aid.
  • Notifiable incident: Any incident that is to be reported to a Regulator including NT WorkSafe, NT Environmental Protection Authority, Department of Environment, Parks and Water Security, NT Health, Utilities Commission, Police or other prescribed agency.
